No, I'm not going to pontificate about what I think the cause is, because I don't know. I've heard a few theories, I've had a few ideas, but all of them seem to be over-simplifications. However, I did just read a post from someone who has the background knowledge to understand things better than I do, and the ability to write eloquently, so let me spread the link.
